If there is an address change, please contact the following applicable parties to ensure your records are correct:

• Sandia Labs- Please contact HR Solutions by email or by phone at 1-505-844-4237. You can also go to the Sandia Health, Benefits & Employee Services website.
• Via Benefits Medical and Dental Benefits. Please call 1-888-598-7809.
• Pre-Medicare Retirees can also go to www.sandiaretireebenefits.com
• Medicare Retirees can also go to www.my.viabenefits.com/sandia
• Fidelity Investments - manages your 401k. Please call them at 1-800-240-4015 to update your information, or go to www.401k.com
• MetLife Optional Supplemental Life, Dependent Life, and Group Accident Insurance- For assistance changing individual plans, please call the Portability Office at 1-866-492-6983, or go to www.metlife.com/mybenefits.
• Prudential Retirement Services handles your Pension Payments. To change any information, please call them at 1-800-621-1089.
• Prudential Primary Life Insurance- To make changes to your contact information, please call 1-800-778-3827. If you need to change your beneficiary information, follow the instructions below:
• Access the Prudential Beneficiary Designation website at www.prudential.com/mybenefits
• Log in with your User ID and password. If you don't have a User ID or password, click Register Here under "First Time User"
• Enter 90373 in the Control Number field
• Complete the required information and create a personal User ID and password
• Be sure to click SUBMIT on your beneficiary designations when you are making changes
